What to expect during your  consultation

Our initial meeting typically begins by clarifying your dreams and discussing what has been holding you back from accomplishing whatever it is that you desire.

My job as your Life Coach is to create a place of non-judgment and compassion for us to establish a strong dynamic. We then collaborate together to discuss goals specific to your needs.

This is an individualized process, meaning no one has identical experiences, so we will tailor the sessions to fit your unique needs. You are the co-creator of this dynamic and together WE will clarify what are your goals, values, and what matters most to you. THEN, we figure out the best options to address any issues that are not allowing you to live a life full of joy and infinate possibilities.


Our Agreement

This “Agreement” is entered into between yourself as a “Client” and Dr. Judd Johns as a “Life Coach” providing “Life Coaching Services” through Dr. Judd Institute, Inc., a Texas corporation, the “Parent Company.”

Description of Life Coaching Services: Life Coaching is a partnership (defined as an alliance, not a legal business partnership) between Dr. Judd Johns (i.e., Life Coach) and the Client in a thought-provoking and creative process that inspires the client to maximize personal and professional potential. It is designed to facilitate the creation or development of personal, professional or business goals and to develop and carry out a strategy or plan for achieving those goals. 

1. CONFIDENTIALITY
A) Life Coach agrees to maintain the ethics of confidentiality between Life Coach and Client.

B) The Life Coach relationship, as well as all information (documented or verbal) that the Client shares with Life Coach or anyone working alongside Parent Company as part of this working dynamic, will uphold the principles of confidentiality. Be aware that the Client - Life Coach relationship is not considered a legally confidential relationship (e.g., medical, legal, or licensed professions) and thus communications are not subject to the protection of any legally recognized privilege. Life Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent.

Confidential Client Information does not include information that:
(a) was in the Life Coach’s possession prior to it being furnished by the Client; 
(b) is generally known to the public or in the Client’s industry; 
(c) is obtained by the Life Coach from a third party, without breach of any obligation to the Client; (d) is independently developed by the Life Coach without use of or reference to the Client’s confidential information; or
(e) the Life Coach is required by statute, lawfully issued subpoena, or by court order to disclose;
(f) is disclosed to the Life Coach and as a result of such disclosure the Life Coach reasonably believes there to be an imminent or likely risk of danger or harm to the Client or others; and
(g) involves illegal activity. The Client also acknowledges their continuing obligation to raise any confidentiality questions or concerns with the Life Coach in a timely manner. 

C) While some information from Parent Company may be in the public domain, its compilation in a form useful to Parent Company (e.g., curriculum, methods) makes it unique and valuable. Confidential Information may include information created, discovered or developed during the period of or arising out of the clients work with Parent Company, whether before or after the Effective Date of this Agreement. Confidential Information also may include any Parent Company proprietary information, technical data, trade secrets or know-how. At all times, both during the work with Parent Company and thereafter, Client shall keep in the strictest confidence and agree to never disclose or reveal to any person or organization, or use, divulge, publish, report, transfer, or otherwise exploit for your own or anyone’s else’s benefit any of the information discussed without the express written consent of an authorized representative of Parent Company. 

2. SCOPE OF PRACTICE
A) Client understands that although Life Coach holds degrees associated with counseling and psychology, Life Coaching provided by Life Coach or anyone at Parent Company is not mental health counseling, does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.  Client understands and acknowledge that my Life Coach is not acting as a mental health counselor or a medical professional.

B) Client acknowledges that Life Coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association (APA) or the Diagnostic Statistical Manual (DSM) and that Life Coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. Client agrees not to use Life Coaching services in place of any form of therapy. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the Life Coaching relationship agreed upon by the Client and Life Coach.

C) Client understands that Life Coach is not licensed by any Boards and does not perform mental health counseling. Client understands that life coaching is an unregulated industry and that Life Coach is not licensed by the State of Colorado or any other state. 

3. WAIVER & RELEASE OF LIABILITY
Client understands that all services and input offered by the Life Coach are solely for the purpose of aiding Client in achieving his/her goals. Client expressly acknowledges having the ability and capacity to give informed consent, and hereby gives such consent to Life Coach to assist Client in achieving such goals. This agreement is based on the client taking responsibility for creating and implementing their own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from the Life Coaching relationship. As such, the Client releases, waives, acquits and forever discharges Life Coach, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages Client may claim to have or that Client may have arising out of acts or omissions by Client or by Life Coach as a result of the advice given by Life Coach or otherwise resulting from the coaching relationship contemplated by this agreement. Client declares and represents that no promise, inducement or agreement not expressed in this agreement has been made to Client to induce him/her to sign this agreement. This agreement shall bind Client’s heirs, executors, personal representatives, successors, assigns, and agents.

This waiver and release of liability includes, without limitation, all injuries which may occur as a result of;
(a) your use of all amenities and equipment in any facility associated with Parent Company and your participation in any activity, course, program, personal training or instruction,
(b) the sudden and unforeseen malfunctioning of any equipment (c) any and all instructors associated with Parent Company being independent contractors or otherwise, training, supervision, or dietary recommendations. You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability. By signing this agreement, you acknowledge that you understand its content and that this release cannot be modified orally.

(c) to the extent the work between Client, Life Coach, and the Parent Company involves career or business advice, Client acknowledges that Life Coach is not responsible for business outcomes included but not limited to clientele issues, profitability and or business success.

4. SERVICE & FEES
Life Coach will provide every potential client with a free 20 minute consultation by either phone, Skype, or in person. Once an agreement has been made to continue, every client must agree to a 10 session minimum to be completed within a year at the rate of $150 an hour. Upon completion of the 10 session minimum, each individual or couple's session is $150.

Life Coach is available for scheduled phone and Skype sessions at the rate of $150. Weekly “check-in’s” or any correspondence (e.g., email) with Life Coach are $2.50 per minute for the first 20 minutes, after which you will be charged for a full session at the rate of $150. You will always be informed in advance of any change in fees. Parent Company accepts cash, check, and all major credit cards.

5. CLIENT RESPONSIBILITY 
A) Client acknowledges that Life Coaching is a comprehensive process that may involve different areas of your life, including work, finances, relationships, education, and your overall health. The Client agrees that deciding how to handle these issues, incorporating Dr. Judd’s Life Coaching principles into those areas and implementing choices is exclusively the Client’s responsibility. 

B) The Client understands that to enhance the Life Coaching relationship, the Client agrees to communicate honestly, be open to feedback and assistance and to create the time and energy to participate fully in the curriculum provided by Parent Company. 

C) It is the Client's responsibility to notify Life Coach no less than 48 hours in advance for cancellation of any scheduled calls or meetings. Life Coach reserves the right to bill the client for a missed appointment and will attempt in good faith to reschedule.

D) Client further acknowledges that they may terminate or discontinue the Life Coaching relationship at any time. Client agrees to compensate the Life Coach for all Life Coaching services rendered through and including the effective date of termination of the Life Coaching relationship. Specifically, the client will be expected to fulfill the monetary agreement left on the contract if termination is made prior to completing a 10 session minimum.

This document reflects the entire agreement between Parent Company and the Client, and reflects a complete understanding of the parties with respect to the subject matter. This Agreement supersedes as well as terminates all prior written and oral representations. This Agreement may not be amended, altered or supplemented except in writing signed by both the Life Coach and the Client.
